Please fill out the following form no later than one month prior your vacation starting date. Vacation requests are accepted only for June and July. Your tuition will be prorated and you will not be charged for the lessons that fall on the vacation days listed in this form if submitted on time. Any other missed lessons will be considered absences. If a student is on vacation for more than two consecutive weeks we can not guarantee his/her lesson slot can be held.
